Health Screening Questionnaire
The health and safety of our members and guests is our top priority. We ask everyone who attends our Services or visits our Church to provide information regarding any symptoms of, or exposure to COVID-19, with this simple screening questionnaire. Have you tested positive for COVID-19 or are you waiting on the results of a COVID-19 test?
Are you isolating or quarantining because you may have been exposed to a person with COVID-19 or are worried that you may have contracted COVID-19?
Are you living with, or have come into close personal contact, or caring for someone who has tested positive for COVID-19 within the last 14 days?
Have you experienced any of the following symptoms in the past 48 hours?
If you are experiencing symptoms or answer YES to any of the questions above, please DO NOT ATTEND our in-person Church Service. Contact your doctor for direction and watch our service online.

According to CDC guidelines, if you are unvaccinated people: unvaccinated or not fully vaccinated facemask is required to attend a full-capacity worship service or sing in an indoor chorus. You must always also stay 6 feet apart. Everyone entering our Church premises must undergo temperature check located in our lobby, answering the above health-related questions and hand sanitization, before proceeding to our sanctuary.
Anyone with a 100.4°F / 32°C temperature or higher in the past 24 hours, or showing symptoms of sickness, or answering YES to any of the Health Screening Questions, will be asked to worship with us online, for the safety of our members, and guests.
We recommend practicing physical distancing seating for unvaccinated people. Fully Vaccinated people are welcome to sit next to each other at your own choice.
For everyone, please practice no-touch-greeting such as waving or elbow-bumping fellow members and guests. Avoid handshaking, hugging and sharing objects.
No outside food or drinks will be allowed within our Church premise.
We ask that everyone must follow these guidelines based on their vaccination status.
